[#1692] fix user method error when not logged in

Make sure $u is defined when doing permission checks for
displaying notification settings for other users. Otherwise
the page doesn't display for logged-out viewers.

Fixes #1692.

Disable logged-out support requests by default

Although important for production sites, allowing logged-out support
requests on development servers can lead to a lot of spam. This change
allows logged-out support requests to be disabled, and disables them in
the default configuration.

Fixes #1630.

Adds an indication that an entry is locked or filtered to notification
e-mails fired for comment and new entry subscriptions.

Add referenced FAQ to volunteer support notifs

Fixes #1715.

If a FAQ was referenced in a Support answer, the ticket update

Refactor and simplify code for printing entry visibility when
submitting a new post or edit. Use global en.dat for translation
strings instead of repeating in various page contexts.

Also fix a couple of small issues in update.bml from recent
commits - use correct user object, and add space before subject.
