Default to $LJ::IS_SSL even when passing in a host option

Because otherwise we just keep forgetting to do it. This takes care of
links in the control strip and in quick reply.

[#1117] Use protocol relative links when possible

* updates sample APPLE_TOUCH_ICON config to use a protocol relative
siteroot

* updates OPENID_SERVER config to use https vs http

Fixes #1117.
